Send the filled form (PDF or Word) to clientservices@dnalabsinternational.com - Pack and ship your evidence. Place evidence in a sealed, tamper-evident container when possible.
Important notes
- Put a printed copy of the completed Case Submission Form inside the outer box.
- Package the evidence in a secure box or shipping container.
- Ship with a traceable carrier (e.g., FedEx, UPS, DHL). We recommend tracking and overnight shipping.
- You may also drop off in person during business hours.
